Marchmont Podiatry Privacy Notice

Marchmont Podiatry is committed to transparency and to complying with its responsibilities under data protection legislation.  This privacy notice sets out important information regarding how we will use your information and your rights under the legislation.  It is important that you read this notice prior to providing your information.
The Marchmont Podiatry is a data controller under data protection legislation.

How we use your personal data
We collect information about you when booking an appointment and when you attend for your treatment.
When you book an appointment our staff will collect you name and phone number. This is so we can contact you regarding your appointment and so we know how to address you when we contact you.
When you attend your first appointment we will ask for your address, other phone numbers and email address. This is so we can contact you regarding any aspect of your treatments and appointments.
We may also want to contact you with information about our services. We will always ask for your permission before we do this.
We collect your date of birth. This helps us to identify you, especially if we need to contact other health professionals regarding your treatment. We will ask you for details regarding your medical history. This is so we can gain a full picture of your foot health and consider any risks that might affect your treatment plan.
We record the details of your treatments and assessments and are obliged under law to keep these medical records for at least 10 years. These records allow us to review your treatments so we can give you the best possible care.
The personal details we hold are confidential and will not be shared with anyone else without your permission. We may ask for permission to share your information with your GP or other health professional if it is part of your treatment plan.

Your Rights
You have certain rights relating to the personal data being processed.  Please see this link to the ICO for more information on your rights (GDPR rights).
You have the right to access your personal data held by Marchmont Podiatry.  Please contact annekidd247@sky.com to exercise this right.
You are entitled to have personal data rectified if it is inaccurate or incomplete.

If you have any concerns/issues with the way the your personal data is processed you can contact the annekidd247@sky.com .  You also have the right to lodge a complaint against Marchmont Podiatry regarding data protection issues with the Information Commissioner’s Office (https://ico.org.uk/concerns/).
